** Summary
* * The Social Worker 2 provides advanced skilled interventions for the support and/or resolution of patient and family crises, problem solving and decision-making advocacy, facilitation necessitated by life-changing events, mediation of risk factors, coping with disease or disability, and other psychosocial and/or socioeconomic issues. Is a resource for the Social Worker 1.
** Education
* * Masters Degree in Social Work or related field required.
** Certification, Registration & Licensure
**** Experience
* * Minimum of three years clinical Social Work in a hospital, Skilled Nursing facility, Home Health or Hospice setting. Demonstrates effective interpersonal, organizational and motivational skills. Demonstrates ability to function as a team player. Proficient in written and verbal communication.
** Responsibilities
* ** Upon consultation, conducts a comprehensive assessment of the patient's behavioral, psychological and  socio-economic needs.
* Attends Care Planning or Discharge Planning rounds to facilitate communication and the importance of  the patient and family's goals.
* Participates in end of life discussions and advance care planning.
* Bases social work actions by code of ethics according to National Association of Social Workers (NASW).
* Documents measurement and evaluation of social service consultation and satisfaction outcomes as  patient progresses towards desired goals.
* Provides leadership to departmental staff.
* Performs other duties as assigned.
