RN Transfer Coordinator - Evening Shift

Wentworth-Douglass Physicians Organization (WDP) seeks a Transfer Nurse Coordinator for a 24-hour evening shift. This role coordinates and facilitates safe transfers of patients within and between facilities. Responsibilities include conducting patient assessments, ensuring documentation, and supporting patients and families throughout the transfer process.

The ideal candidate has a Bachelor's in Nursing, a valid RN license, and 2-3 years of clinical experience, preferably in acute or critical care settings. Excellent communication and critical thinking skills are essential.
