We are a busy and growing family medical practice located on the seacoast of NH looking to add to our team of Nurse Practitioners (APRN) working as Primary Care Providers in a Private Practice setting.
Advanced Degree and Certification in primary care, family medicine required. Our Team of experienced Primary Care Providers are exclusively Nurse Practitioners and while we are open 5 days a week, full time employees work 4 days a week. We cannot offer nights or weekend shifts.
Job Responsibilities:
- Complete assessment of patient’s health status and thorough documentation in patient’s chart (EHR)
- Include relevant diagnostic testing with correct interpretation
- Assure that patient is notified of results
- Develop a mutual plan of care with patient including alternative medical course of action
- Highlight prevention and health maintenance aspects for best outcome scenario
- Develop a therapeutic relationship with patients
- Maintain professional decorum allowing opportunity for objectivity
- Refer to and use the NH formulary when prescribing medications
- Assure that patient documentation is complete after each visit and that diagnostic testing is ordered as needed
- Utilize appropriate referral contacts as provided
- Maintain infection control standards while delivering care
- Cultivate and assist in specific patient and staff education courses
- Maintain productive guidelines provided by management
- Assure professional growth by attending appropriate external clinical educational sessions.
- Consult with Medical Director when patient care warrants
- Actively display team spirit and foster teamwork with all staff
- Participate in staff meetings and chair a committee (if appropriate)
- Actively participate in quality improvement process
- Maintain patient confidentiality and adhere to HIPAA guidelines
- Other responsibilities as needed
Qualifications:
- Registered Nurse with advanced Master’s level preparation. Licensed as ARNP in the State of New Hampshire
- Experienced in Family Medicine as an ARNP with ability to care for all ages
- Basic Life Support Certification
- At least five years of experience in ambulatory setting as an Independent PCP
- Ability to be a positive team member possessing creative problem-solving skills
- Excellent interpersonal skills
- Show evidence of advanced organizational skills
- Demonstrated record of reliability
Knowledge / Skills / Abilities:
- Ability to receive detailed information through oral communication.
- Ability to express or exchange ideas by means of the spoken word.
- Ability to interact effectively with people of varied educational, socioeconomic and ethnic backgrounds, skills levels and value systems; to work with frequent interruptions and to respond appropriately to unexpected situations. Excellent listening skills.
- Visual acuity sufficient for work which deals largely with preparing and analyzing clinical data and patient information.
- Excellent written, verbal communication and social skills, computer skills.
