Wentworth-Douglass Hospital is seeking a full-time SUD Care Coordinator to work on-site in Dover, NH, Monday through Friday from 8:00am to 5:00pm. You will help patients navigate recovery, coordinate services, and connect them with appropriate resources as part of the continuum of addiction treatment.
The role involves collaboration with a multidisciplinary team, mentoring and advocacy for recovery pathways, and supporting medication-assisted treatment in a recovery clinic.
