The RN Case Manager coordinates and facilitates the care of the patient population through effective collaboration and communication with the Interdisciplinary Care Transitions (ICT) team members. This role involves following patients throughout the continuum of care and ensuring optimum utilization of resources, service delivery, and compliance with external review agencies. Responsibilities include providing ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs, enhancing the quality of patient management and satisfaction, and promoting continuity of care and cost-effectiveness through the integration of case management functions.
Responsibilities- Coordinate patient care in collaboration with ICT team members.
- Follow patients through the continuum of care.
- Ensure compliance with external review agencies.
- Conduct comprehensive patient assessments and develop care plans.
- Evaluate individual patient needs and implement care plans effectively.
- Enhance patient management quality and satisfaction.
This position is full-time, Monday to Friday, from 8:00 AM to 4:30 PM.
Required Experience / Certifications / Licensure- Minimum of 4 years of recent RN Case Management or Licensed Clinical Social Worker experience.
- Experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
- Knowledge of MCG and discharge planning across acute care settings.
- Strong interdisciplinary team collaboration skills.
- Current New Jersey RN license or multi-state RN license or New Jersey Social Worker license.
- BLS certification through the American Heart Association must be attached to the submission packet.
