JOB TITLE:

Home Health RN Supervisor / RN Case Manager (RNCM)

We are seeking a highly skilled, experienced, accountable, and self‑motivated home health RN Case Manager (RNCM) to lead client services and support a multidisciplinary team overseeing care for existing and returning clients throughout Northeast Ohio.

Responsibilities
  • Coordinate case management, starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services.
  • Conduct occasional new hire orientations, in‑service, and supervisory reviews.
  • Provide input to influence training, education, care practices, outcomes, and business success.
  • Backup skill‑level visits as necessary.
  • Perform client assessments and direct, plan, and initiate appropriate actions in home‑care situations.
  • Oversee a team of RNs, LPNs, Home Health Aides (HHA), and Personal Care Aides.
  • Document, communicate, and coordinate patient status or changes with the care team.
  • Promote personal safety and a safe environment for clients and coworkers.
  • Travel surrounding counties as required.
Schedule & Service Area
  • Part‑time RNCM – Minimum travel radius of Tuscarawas and Coshocton counties plus surrounding areas up to 60 minutes.
  • Full‑time RNCM – Region includes Tuscarawas, Coshocton, Stark, Summit, Cuyahoga and surrounding counties up to a 2‑hour travel radius. Additional cases may be added if desired.
  • Qualified applicants may reside in or near any primary service area if travel requirements are met.
Schedule Type
  • Flexible and customizable; we build a schedule close to home that fits availability and preferences.
  • Minimum availability:
    Monday–Friday (weekdays) 8 am–5 pm.
  • No on‑call, overnight shifts or overnight travel required.
  • Weekend services, overtime, respite, and PRN offered but not required.
Employment Type

Long‑term employment for full‑time or part‑time positions. No on‑call, overnight shifts or overnight travel required. Weekend services optional.

Compensation
  • Part‑time and full‑time hourly wage: $35–$40 per hour.
  • Full‑time salary starts $72,800–$83,200 per year with quarterly bonus opportunities (requires pre‑determined minimum visits/servicies per week).

Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse with an active RN license in Ohio.
  • CPR and First Aid certification required.
  • 1 year experience in case management, with home health experience highly preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record.
  • Desirable traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful, positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current driver’s license and reliable transportation.
  • Minimum of one year nursing supervisory experience (preferably in home care).
Job Summary

As an RN Case Manager, you will coordinate and provide patient care, ensuring optimal outcomes. With core skills in managed care, patient assessment, client care documentation, and discharge planning, you will oversee services and provide expert nursing care. You will also manage a multidisciplinary team, ensuring effective communication among physicians, agency staff, service administrators, other caregivers, clinicians, the client, and family.
