Home Health RN Supervisor / RN Case Manager (RNCM)
We are seeking a highly skilled RN Case Manager (RNCM) to manage and coordinate care for clients in Northeast Ohio. The position offers a structured schedule, opportunities for advancement, and competitive compensation.
Job SummaryAs an RN Case Manager, you will coordinate patient care, conduct assessments, communicate with physicians and agency staff, supervise a multidisciplinary team, and ensure high‑quality home‑health services.
Responsibilities & Requirements- Perform client assessments, direct, plan, and initiate appropriate actions in home‑care situations.
- Supervise a team of RN, LPN, Home Health Aide, and Personal Care Aides.
- Document, communicate, and coordinate patient status changes with the care team.
- Promote personal safety and a safe environment for clients and coworkers.
- Travel as required within the service region.
- Minimum of one year of nursing supervisory experience; home‑care experience preferred.
- Graduate of an accredited School of Nursing.
- Minimum of one year experience as a Registered Nurse, with an active RN license in Ohio.
- CPR and First Aid certification (or obtainable prior to hire).
- One year of case‑management experience; home‑health experience highly preferred.
- Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
- Strong background in nursing, patient care, and confidentiality.
- Strong attendance and dependability record.
- Familiarity with HIPAA regulations and medical terminology.
- Current driver’s license and reliable transportation.
- Part‑time hourly wage: $35–$40 per hour.
- Full‑time salary: $72,800–$83,200 per year, with quarterly bonus opportunities.
- Round‑trip mileage reimbursement.
- Flexible, customizable schedules with support from the in‑house Scheduling Team.
- Eligible for quarterly bonuses, career advancement, new‑employee and client referral bonuses.
- Part‑time RNCMs receive hourly pay, weekly pay, and quarterly bonus opportunities.
- Full‑time RNCMs may choose hourly or salaried pay; eligible for PTO, health, dental, vision coverage, and other benefits.
Employment Type
- Long‑term employment; works part‑time or full‑time.
- No on‑call, no overnight shifts, overnight travel; weekend services are optional.
- Minimum availability:
Monday–Friday, 8 a.m.–5 p.m.; schedule built close to home. - Overtime, respite, and PRN services offered when available.
- Part‑time RNCM:
Tuscarawas, Coshocton counties and surrounding areas up to 60 minutes. - Full‑time RNCM:
Tuscarawas, Coshocton, Stark, Summit, Cuyahoga counties and surrounding counties up to a 2‑hour travel radius. - Qualified applicants may reside anywhere within the primary service areas if travel requirements are met.
