Overview

Graham Healthcare Group is seeking a Home Health Physical Therapist for a job in Downers Grove, Illinois.

• Collaborate to provide management and delivery of patient care plans in a team-based home health setting.
• Support patients in Downers Grove and surrounding areas; eligible for $20,000 total bonus potential (including a $10,000 first-year success bonus and a $10,000 sign-on bonus paid within the first 18 months of employment).
• Maintain high-value rewards through performance-driven compensation and benefits; adhere to established schedules and meet critical deadlines for charting.
• Utilize tablets, mobile phones, and EMR software; demonstrate self-management of time and scheduling; participate in ongoing training and professional development.
• Utilize company-provided devices; participate in after-hours care when required; engage with schedulers to support flexible scheduling options.
• Drive to patient locations as needed (approximately 20% of the workday).

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college (required).
• Licensed Physical Therapist issued by the state of employment.
• Minimum of one year of healthcare experience.
• Current driver’s license and ability to travel to patient locations.
• Commitment to meet critical deadlines for charting; ability to self-manage time and schedule.
• Experience with tablets, mobile phones, and EMR software.
