As a Physical Therapist Assistant in home health
, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility
.
- Provide in-home physical therapy services following the established Plan of Care
- Assist patients with therapeutic exercises, mobility training, and rehabilitation programs
- Support senior care patients in improving strength, balance, and independence
- Monitor, document, and report patient progress to the supervising Physical Therapist
- Educate patients and caregivers on home exercise programs, safety, and fall prevention
- Collaborate with physicians, RNs, and the interdisciplinary care team
- Complete documentation using Well Sky Kinnser EMR
- Active California Physical Therapist Assistant (PTA) license
- Current CPR certification (AHA required)
- Minimum 1 year of clinical experience (home health experience preferred)
- Strong interest in home health, rehab, and senior care
- Excellent communication and organizational skills
- Valid driver’s license, auto insurance, and reliable transportation
- Flexible PTA home health schedule and independence
