What You’ll Do- Deliver hands‑on nursing care in the patient’s home
- Monitor vitals, manage medications, and follow the care plan
- Provide comfort, safety, and support to the patient and family
- Document care accurately and promptly
- Active LVN license
- Current CPR certification
- Tracheostomy and Ventilator certification
- Compassion, professionalism, reliability
- Home health or shift care experience a plus
- Flexible shifts (days, nights, weekends)
- Supportive clinical leadership
