Job Summary and PurposeUnder the direct supervision of the Program Manager, and working in collaboration of the Medical Director, Clinical Director and MAT AOD Specialist, the Licensed Vocational Nurse (LVN) provides professional nursing services for this residential substance use program. As a patient advocate, the LVN will also provide limited physical examinations, medication support, detox withdrawal management care, and MAT educational services.
Clients are assessed using the full American Society of Addiction Medicine (ASAM) assessment and receive the following services: education and counseling groups, individual counseling sessions with a Substance Abuse Specialist and/or a therapist, case management, transportation, and random drug screening.
Duties and Responsibilities- Provides nursing services including monitoring of withdrawal symptoms, monitoring of medications including MAT, monitoring vitals, direct communication with treatment medical providers on or off-site for referrals to other providers and community health resources, health instruction to patients, and instruction and in-servicing to counseling line staff including monitoring of self‑administration of prescribed medication.
- Designs and provides a comprehensive health services plan for each client that includes a broad range of medical services including assisting clients with physical and emotional needs, and referral to appropriate service providers when needed.
- Documents patient’s current medication list, including name of prescriber, names of medication, dosages, frequency, duration and start date.
- Participates in weekly case review with Program Staff to ensure best quality care for clients and continuous improvement of services.
- Acts as liaison between Program Staff and Medical Providers, in order to coordinate best care of clients.
- Maintains electronic health records and hard copy documentation of all services provided.
- Other tasks that may be required by the Program Manager.
- Services for this position must be provided on site; the position requires incumbents to report to work on site each day of active employment.
- Assign case load of LVN Team, mentor, monitor services.
- Provide on‑the‑job training, facilitate ongoing professional training of LVN team.
- Provide nursing care to program clients.
- Design health services plan for program clients, coordinate care with primary and other healthcare providers.
- Monitor clients’ self‑administration of prescribed medication, maintain documentation of all medications.
- Weekly review of clients’ care with program staff, coordinate reviews with primary care providers.
- Liaison between Program and primary care providers.
- Maintain accurate health records of clients.
- Other duties as required and subject to change at any time.
- Must be able to pass a fingerprint clearance, background check, including criminal history, personal references, employment and education verifications.
- Ability to perform under circumstances of possible emotional stress and conflicts dealing with difficult, uncooperative, and potentially aggressive clients.
- Current and valid California Licensed Vocational Nurse.
- Valid Cardiorespiratory Resuscitation (CPR) certification.
- Must have completed ASAM I and ASAM II for access to EHR.
- One year of work experience as an LVN.
- Work experience serving substance abuse clients.
- Culturally sensitive and demonstrated ability and effectiveness working with mental health and substance use disorder.
- Experience working with the homeless population.
- Bilingual (English/Spanish – written and oral).
