Summary:

• Responsible for performing a variety of duties depending on assignment. These duties include, but are not limited to: direct patient care, triaging phone calls, and patient outreach, and care coordination, clerical, environmental and organizational tasks

Responsibilities:

• Assess, plan, implement, document, coordinate, and manage patient care at the direction of the physician giving consideration to age-specific accommodations.
• Provide patient care, which demonstrates a comprehensive knowledge of normal physiology and disease pathophysiology, appropriate nursing and medical interventions and a knowledge of the principles of growth and development over the lifespan.
• Implement patient pre and post care based on evidence based practice.
• Document patient response to treatment and effectiveness of care.
• Perform rooming and triage functions according to patient's age and reason for visit.
• Identify gaps in care and perform patient outreach to close those gaps.
• Provide teaching and discharge planning based on assessment of self-care knowledge/ability on an ongoing basis.
• Insure appropriate completion of chart in eClinical Works and other documentation.
• In collaboration with the physicians able to intervene as appropriate to prevent complications.
• Demonstrates initiative and self-direction in completion of assigned responsibilities in accordance with department needs.
• Obtains necessary pre authorizations and pre certifications for testing as required by patients insurance.
• Performs selected therapeutic interventions and diagnostic procedures according to department standards as ordered to include but not limited to: point of care testing ¨ application and removal of wound vacs and other wound appliances ¨ specimen collection ¨ suture removal ¨ wound packing/removal ¨ assists surgeon during vein closure procedures, using sterile techniques ¨ 2 patient identifiers will be utilized before performing these interventions and/or procedures.
• Assists patients physically by providing physical support through pushing, pulling, lifting, reaching, turning, kneeling, standing, sitting and balancing, and psychologically by providing emotional intervention.
• Demonstrates ability to perform with clinical proficiency in crises and medical emergencies.
• Communicates and collaborates with patients, physicians, family members, and other health care disciplines in order to achieve excellence in patient care during hospitalization.
• Promotes patient safety through adherence to protocols, policies, procedures, evidence-based guidelines and OSHA Patient Safety Goals.
• Actively participates in and supports departmental and organizational quality initiatives and goals.
• Maintains clinical competency according to unit-specific standards.

Credentials:

• Licensed Practical Nurse (Required)
• Basic Cardiac Life Support (Required)

Education or Equivalent

Experience:

• Other
• Graduate of an accredited School of Practical Nursing. 1-2 years experience in direct patient care Successful completion of appropriate continuing education specific to job requirements
