Summary:

+ Responsible for performing a variety of duties depending on assignment. These duties include, but are not limited to: direct patient care, triaging phone calls, and patient outreach, and care coordination, clerical, environmental and organizational tasks

Responsibilities:

+  1. Assesses, plans, implements, documents, coordinates, and manages patient care at the direction of the physician giving consideration to age-specific accommodations.
a. Maintains a computerized, evolving plan of care based on nursing assessment, the medical plan of care and patient need. The plan is initiated according to department standards.
b. Provides patient care, which demonstrates a comprehensive knowledge of normal physiology and disease pathophysiology, appropriate nursing and medical interventions and a knowledge of the principles of growth and development over the lifespan.
c. Implements patient pre and post care based on evidence based practice.
d. Documents patient response to treatment and effectiveness of care.
e. Performs rooming and triage functions according to patient's age and reason for visit.
f. Identifies gaps in care and performs patient outreach to close those gaps.
g. Provides teaching and discharge planning based on assessment of self-care knowledge/ability on an ongoing basis.
h. Insures appropriate completion of chart in eClinical Works and other documentation.
i. Insures that no unapproved abbreviations are used when documenting on patient's medical record.
j. In collaboration with the physicians able to intervene as appropriate to prevent complications.
k. Demonstrates initiative and self-direction in completion of assigned responsibilities in accordance with department needs.
l. Obtains necessary pre authorizations and pre certifications for testing as required by patients insurance   m. Performs selected therapeutic interventions and diagnostic procedures according to department standards as ordered to include but not limited to: point of care testing ¨ application and removal of wound vacs and other wound appliances ¨ specimen collection ¨ suture removal ¨ wound packing/removal ¨ assists surgeon during vein closure procedures , using sterile techniques ¨ 2 patient identifiers will be utilized before performing these interventions and/or procedures   n.

Assists patients physically by providing physical support through pushing, pulling, lifting, reaching, turning, kneeling, standing, sitting and balancing, and psychologically by providing emotional intervention.
o. Demonstrates ability to perform with clinical proficiency in crises and medical emergencies.
2. Communicates and collaborates with patients, physicians, family members, and other health care disciplines in order to achieve excellence in patient care during hospitalization.
a. Utilizes effective interpersonal skills including interaction with staff through understanding, talking, and hearing, seeing and listening.
b. Involves patient/significant other in all aspects of the plan of care, serving as advocate when necessary.
c. Ensures all procedures that require insurance precertification's are completed and all departments have the necessary documentation   d. Consistently and continuously communicates, consults and shares pertinent information regarding changes in patient status and patient care.
e. Uses interpersonal communication techniques that show a willingness to accept and give constructive feedback for the purpose of promoting professional excellence.
