Sleep Technologist Patient Care Coordinator

Forward Health LLC seeks a Sleep Technologist Patient Care Coordinator to support evidence-based mental health and sleep care. In this role, you will conduct and monitor sleep studies, score data, and assist providers in diagnosing and treating sleep-related conditions that impact mental wellness. You will coordinate patient scheduling, intake, and follow-up, ensuring clear communication and a compassionate, patient-centered experience. Working closely with psychiatrists and therapists, you help implement integrated care plans, educate patients on sleep hygiene and equipment use, and maintain accurate, compliant clinical documentation in a collaborative, learning-focused environment.
