Physical Therapist I-Per Diem

- Screen patients to determine need for intervention and initiate referrals as appropriate.
- Evaluate patients using appropriate age‑specific tools and interpret data to plan treatment.
- Establish goals and treatment plans based on evaluation data, rehabilitation potential, discharge plans, and in coordination with goals and plans of patient, family and other care team members.
- Demonstrate skill and creativity in implementing patient treatment programs which are safe, effective, and efficient.
- Modify treatment program according to patient needs and progress, terminate treatment appropriately, and develop appropriate discharge and post‑discharge plans.
- Complete evaluations, progress notes, discharge summaries, and third‑party reimbursement reports per established departmental requirements.
- Perform reassessments per established departmental protocols as needed.
- Collaborate with physicians and other healthcare professionals to ensure ongoing integrated patient care.
- Establish and maintain therapeutic relationships with patient and caregivers.
- Attend team and family conferences; interpret pertinent findings and contribute to the development of a multidisciplinary plan of care.
- Doctor of Physical Therapy from an accredited school or equivalent master’s degree.
- Entry‑level position; no experience required, but interest in oncology preferred.
- Current Physical Therapist license in the State of California or PTLA status in California.
- BLS Certification through the American Heart Association.
- For pediatric service, CCS Panel provider or complete application within 60 days of hire or when hours attained.
Hourly Per Diem Rate: $58.80
City of Hope employees pay is based on the following criteria: work experience, qualifications, and work location.
