Summary
Under the supervision of the Supportive Housing Manager, the Case Manager oversees the delivery of resident services programming at properties serving high‑needs individuals, families, seniors, and those with special needs. The role coordinates services for high‑needs populations and low‑income households, and develops collaborative relationships with county providers and external services to deliver robust programming.
Essential Duties and Responsibilities- Independently deliver onsite programs and ensure they meet internal standards, objectives, and regulatory expectations.
- Conduct door‑to‑door, phone, and e‑mail outreach to inform residents of available services.
- Provide crisis intervention and follow‑up, including mandatory reporting for child or adult abuse.
- Support case‑management services for residents with functional impairments and facilitate the development of independent living skills.
- Provide group and one‑on‑one services, and support community‑building activities.
- Collaborate with residents to support independent living and personal and professional goals.
- Conduct needs assessments and community meetings to understand resident needs and guide them toward appropriate services.
- Survey the resident population, identify priority needs, and solicit resident input on service preferences.
- Participate in pilot program development and implement new service models at assigned properties.
- Collect data for all programming, record information in Salesforce, and submit accurate reports promptly. Maintain all reporting requirements set by funding or governmental agencies.
- Evaluate outcomes of onsite programs and services for quality improvement.
- Educate residents about programmatic options and advocate for their needs.
- Ensure all HIPAA policies and procedures are maintained and adhered to.
- Understand partnership goals and criteria, engage with property‑level partners, and communicate improvements to supervisors.
- Collaborate with local community agencies near the assigned property.
- Work with Eden Property Management, Eden Resident Services, community partners, and service providers to address and resolve property concerns or neighbor disputes.
- Support property‑level needs assessments and follow‑up, including community meetings.
- Participate in developing responsive property service plans.
- Communicate and execute actions based on assessed needs.
- Deliver exemplary customer service and responsiveness to residents.
- Maintain accurate property‑level records and files, entering and tracking all service activities daily, weekly, and monthly.
- Ensure timely property‑level reporting consistent with Services’ policies and procedures.
- Participate in staff and team meetings, trainings, group outings, and site‑sponsored events.
- Notify the direct supervisor of activities and incidents promptly.
- Cultivate collaborative relationships with Property Management peers and colleagues at the property level.
- Perform other duties as assigned.
Education and Experience
- Bachelor’s degree in social work, psychology, counseling, or a related field (required);
Master’s degree in the same fields is preferred. - At least 3 years of experience serving senior populations and special needs (mental illness, substance use, co‑occurring disorders).
- Experience working with individuals and families with intellectual developmental disabilities.
- Experience using and administering multiple assessment tools and interpreting assessment data.
- Experience supporting program and service implementation and coordination.
- Experience collaborating with local service providers.
- Excellent reading, writing, and verbal communication skills; comfortable communicating across language barriers, including using translation services. Bilingual requirements may be determined by resident population.

- Excellent relationship‑building, active listening, and conflict‑resolution skills.
