Southland Healthcare and Rehab is currently seeking compassionate, dependable, and motivated Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our nursing team for full‑time 12‑hour shifts. • Provide direct nursing care to residents in accordance with care plans, physician orders, and facility policies.
• Administer medications and treatments accurately and on time.
• Monitor residents for changes in condition and report concerns appropriately.
• Assess residents and communicate relevant findings to physicians, families, and the interdisciplinary team.
• Supervise and support Certified Nursing Assistants and other assigned staff.
• Ensure proper documentation of care, treatments, incidents, and resident status.
• Respond promptly and professionally to resident needs, emergencies, and call lights as needed.
• Review, implement, and update resident care plans as appropriate.
• Maintain infection control and safety standards throughout the facility.
• Participate in admissions, discharges, transfers, and other resident care coordination activities.
• Provide resident and family education when needed.
• Work collaboratively with team members to promote positive clinical outcomes and resident satisfaction.
• Ensure nursing services are delivered with dignity, compassion, and respect.

• Current and active LPN or RN license in the State of [Insert State].
• Current CPR certification preferred.
• Previous experience in long‑term care, skilled nursing, rehabilitation, or post‑acute care preferred.
• Strong clinical assessment and critical thinking skills.
• Ability to supervise staff and work effectively as part of a team.
• Excellent communication and documentation skills.
• Dependable attendance and professional conduct.
• Ability to work 12‑hour shifts and meet the physical demands of the position.
• Must pass required background screening and any additional pre‑employment requirements in accordance with facility policy and state regulations.

• Experience with medication administration and treatment documentation.
