Description:

This is a contract assignment for a Home Health LPN.The assignment duration is 13 weeks.

The schedule includes Monday through Friday from 8 AM to 5 PM.The clinician will be on call one week per month, including weekends.

The role involves visiting patients in both care facilities and their homes.

There is a possibility of hiring the contractor at the end of the assignment.

Clinicians will need to travel 30 to 40 miles a day, with weekly travel ranging from 50 to 60 miles.

Average daily caseload is approximately 5 to 10 patients per day.

Weekly productivity expectations are set at 30 points, which may equate to 40 hours per week.

Contractors typically see around 6 to 8 patients daily.

Documentation of visits must be completed within 24 hours.

There are no guaranteed hours with this contract.

If patient census is low, clinicians may have the opportunity to float to nearby locations for additional hours.

Case types may include start of care, wound care, and follow-up visits.

Clinicians must be comfortable with traveling up to 75 miles from their assigned clinic.

Reliable transportation, a current driver’s license, and agency-required liability insurance are required.

Mileage between patient homes is reimbursed at the IRS standard rate.

All documentation must be completed in a timely manner to ensure patient care and billing.
