The Community Service Board of Middle Georgia's Intensive Case Management (ICM) program is seeking a qualified individual to serve as our next ICM Team Lead. Intensive Case Management (ICM) consists of providing environmental support and care coordination considered essential to assist a person with improving his/her functioning, gaining access to necessary services, and creating an environment that promotes recovery as identified in his/her Individual Recovery Plan (IRP).
The focus of the interventions includes assisting the individual with:
1) developing natural supports to promote community integration;
2) identifying service needs;
3) referring and linking to services and resources identified through the service planning process;
4) coordinating services identified on the IRP to maximize service integration and minimize service gaps; and
5) ensuring continued adequacy of the IRP to meet his/her ongoing and changing needs. ICM’s overall goal is to reduce recidivism and hospitalization, gain stability, provide an understanding of symptoms and behavioral health diagnoses, and promote continuity of care. ICM Case Managers have a maximum of 20 individuals in an expected caseload, but they will be meeting the same individuals every week until their case is closed.

- Provide day-to-day supervision, coaching, and support to ICM staff
- Conduct and document weekly treatment team meetings, ensuring each enrolled individual is reviewed at least monthly
- Monitor staff productivity, documentation quality, service delivery, and compliance with program standards
- Assist with onboarding, mentoring, and ongoing development of ICM staff
- Collaborate with leadership to identify operational needs, barriers to service delivery, and opportunities for continuous improvement
- Ensure individuals are contacted within five (5) business days of referral and successfully engaged in services
- Monitor compliance with all DBHDD Intensive Case Management standards, including required face‑to‑face contacts, community‑based service delivery, documentation timelines, and billing requirements
- Ensure all required assessments, housing documentation, treatment plans, crisis plans, and service authorizations remain current and compliant
- Monitor engagement efforts and ensure assertive outreach strategies are utilized for difficult‑to‑engage individuals
- Review and address gaps in documentation, service delivery, or compliance to reduce audit findings and improve program outcomes
- Oversee coordination of behavioral health, medical, housing, financial, vocational, and community‑based services
- Ensure individuals receive assistance obtaining housing, benefits, transportation, and other community resources identified within their Individual Recovery Plan (IRP)
- Coordinate care transitions for individuals discharging from psychiatric hospitals, crisis stabilization units, correctional facilities, or episodes of homelessness
- Promote continuity of care through collaboration with providers, family members (when appropriate), community agencies, and natural supports
- Ensure services are delivered in the individual's preferred environment whenever possible, including homes, community locations, shelters, healthcare facilities, or other appropriate settings
