National Church Residences is seeking a Transition Coordinator to support adults moving from long-term care facilities to home or community-based settings in Northeast Ohio. The role is on-site, Monday–Friday, with travel across Delaware, Licking, Union, Madison, Pickaway, Fairfield and surrounding counties.
You will work with healthcare and housing professionals to coordinate services, ensure person-centered transitions, and uphold client rights and dignity. A valid driver’s license is required.
