Option Care Health is seeking a Clinical Transition Educator to provide clinical education and training for successful patient transitions to our care model. The role partners with the sales team to grow patient transitions to OCH.
The educator will conduct patient assessments, coordinate with hospital staff and internal teams, and ensure timely discharge, therapy education, and continuity of care. Local travel is required across care facilities.
