Suncrest Hospice - - Responsibilities:
Provide resident care to assigned residents by performing assessments and implementing care plans under the direction of a Case Manager;
Collaborate with licensed care staff to coordinate appropriate care for residents;
Follow all nursing policies, procedures, and safety guidelines;
Work closely with patients on a 1-1 basis with support from the patient care team;
Communicate and collaborate with nurses, social workers, chaplains, volunteers, and STNA leadership
