- Provide basic nursing care to residents, including administering medications, wound care, and assisting with personal hygiene.
- Monitor residents’ vital signs such as temperature, blood pressure, and pulse, reporting any abnormalities to the supervising nurse or physician.
- Assist with feeding, dressing, and mobility, ensuring residents’ comfort and safety.
- Follow individualized care plans and provide care according to residents’ medical conditions.
- Administer medications to residents as prescribed by physicians, ensuring accurate documentation and timely delivery.
- Monitor residents for adverse reactions to medications and report any concerns.
- Maintain accurate records of medication administration and residents’ responses to treatments.
- Maintain up-to-date and accurate documentation of patient care, including vital signs, treatments, and any changes in condition.
- Report residents’ progress, concerns, or changes in health status to the supervising nurse or physician.
- Ensure that all documentation complies with facility policies and healthcare regulations.
- Work closely with RNs, physicians, and other healthcare professionals to ensure cohesive patient care and effective communication.
- Communicate with residents and their families, addressing concerns and providing updates on residents’ health and treatment.
- Participate in care conferences and interdisciplinary team meetings to discuss resident care plans and outcomes.
- Follow infection control protocols and maintain a clean and safe environment for residents, staff, and visitors.
- Use personal protective equipment (PPE) appropriately and ensure compliance with facility safety guidelines.
- Report any safety hazards or concerns to the appropriate personnel.
- Act as an advocate for residents, ensuring their needs are met and their dignity is respected.
- Provide emotional support to residents and their families, offering comfort and reassurance when needed.
- Promote a positive and compassionate environment for all residents, fostering their physical and emotional well-being.
- Education:
Completion of an accredited LPN or LVN program. - Licensure:
Current and active Licensed Practical Nurse (LPN) license. - Experience:
Previous experience in a healthcare or long‑term care setting is preferred but not required. New graduates are welcome to apply. - Strong clinical and patient care skills.
- Excellent communication and interpersonal abilities.
- Ability to work well as part of a team and collaborate with other healthcare professionals.
