RN Case Manager – Weekend Cover

Employment Type:

Part time. Shift: 8‑hour shift each week.

About The Job

Our RN Case Manager collaborates with other internal and external disciplines to direct and coordinate the care of patients from admission to transfer or discharge, aiming for cost‑effective utilization of hospital resources, minimizing out‑of‑pocket expenses for patients, and providing customer support.

What You Will Do
  • Identify discharge needs, determine the appropriate level of care for patient transition, coordinate planning, and solve continuity issues to ensure seamless continuum of patient care and prevent readmission.
  • Ensure all appropriate clinical information is provided to outside agencies.
  • Communicate pertinent information to the appropriate members of the patient care team both effectively and professionally.
  • Identify and eliminate barriers to effective patient stay management and promote improvements in work processes within the department, hospital, and community.
  • Be responsible for professional development, maintaining licensure, and self‑education.
  • Advocate for patient legal, social, and ethical rights while balancing patient resources and hospital financial stewardship.
  • Refer quality issues and poor utilization of services to the appropriate sources.
  • Coordinate/facilitate patient/family education.
What We Are Looking For
  • Associate degree from an accredited RN program
  • Current license to practice as a registered nurse in the State of Ohio
  • Minimum of 3–5 years of recent clinical experience in an acute care setting
