The Registered Nurse plans, organizes, and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. The role offers one-on-one patient care, flexible scheduling, autonomy, and paid mileage and drive time between patient homes.
EssentialJob Functions / Responsibilities
- Patient Care
- Complete an initial assessment of patient and family to determine home care needs. Provide a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluate patient nursing needs.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan, establishing goals based on nursing diagnosis and incorporating therapeutic, preventive, and rehabilitative nursing actions. Include the patient and family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
- Counsel the patient and family on meeting nursing and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
- Communication
- Prepare clinical notes daily and update the primary physician when necessary and at least every sixty days.
- Communicate with the physician regarding the patient’s needs, reports, and changes in the patient’s condition; obtain/receive physician orders as required.
- Communicate with community health-related persons to coordinate the care plan.
- Additional Duties
- Participate in on-call duties.
- Ensure arrangements for equipment and other necessary items and services are available.
- Instruct, supervise, and evaluate home health aide care provided every fourteen days.
- Currently licensed as an RN through the State Board of Nursing.
- Possesses and maintains current CPR certification and a valid driver’s license.
- Prefer at least one year of general nursing experience (home health, clinical, surgical, or critical care).
- Is self-directed with the ability to work with little supervision; has good organizational skills.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
- Has ability to walk or stand for prolonged periods; lift, position, or transfer patients; lift supplies and equipment; reach, stoop, bend, kneel or crouch; visual acuity and hearing sufficient to perform required nursing skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or agency requirements and is in good working condition.
- Is flexible and cooperative in fulfilling role obligations.
