Schedule:
Monday–Friday, 8:00 AM – 5:00 PM (Flexible). Includes shared telephonic on-call rotation to support 24/7 clinical stability for high-acuity residents.
Ohio State University Physicians (OSUP) is seeking a Nurse Practitioner to serve as a primary clinical lead within our Skilled Nursing and Sub-Acute Rehabilitation unit. This role is designed for a clinician who excels in a senior living setting, managing complex medical transitions for residents moving from acute hospital care to rehabilitation.
ResponsibilitiesKey Responsibilities
High-Acuity Management:
Provide outpatient care to geriatric patients in a clinic located in Dublin, Ohio. Interdisciplinary Leadership:
Collaborate on patient care with physician, therapy, nursing, and social work.
Clinical Oversight:
Order and interpret diagnostic tests, manage medication reconciliation, and perform minor procedures (i.e. stitch removal) as needed. Perform comprehensive evaluations (H&P, progress notes, (re) certifications).
Quality & Compliance:
Partner with the Medical Director to ensure all documentation meets CMS Five-Star Quality Rating requirements and supports regulatory readiness. Document all patient encounters accurately and timely in the electronic medical record (EMR).
Family Advocacy:
Act as the primary clinical liaison for families, communicating effectively with patients and families to align medical interventions with goals of care.

Licensure:
Active Ohio APRN license and national certification (AANP or ANCC).
Experience:
Minimum of 2 years of experience in hospitalist medicine, geriatrics, or sub-acute Rehab preferred.
Competencies:
Proficiency with Point Click Care or similar EMR; strong autonomous decision-making skills; cohesive collaborator and communicator; ability to thrive in a high(er)-volume, fast-paced environment.
