- Collaborates with clinic team members to provide evidence-based patient-centered care
- Helps patients stay up to date on their care plans
- Focus on high-risk medically complex patients
- Acts as a liaison with patient’s family members and other agencies
- Facilitates improved communication to provide comprehensive whole person care
- Stays up to date on standards, assisting with programs and processes
- Associate Degree in Nursing from a CCNE or ACEN accredited nursing program
- Current Ohio Registered Nurse license
- CPR/BLS certified
- Knowledge of triage procedures
- Clinic, physician office, and/or urgent care experience
- Ability to maintain quality, safety, and/or infection control standards
- Knowledge of appropriate procedures and standards for the administration of medications and patient care aids
- Ability to educate and clearly communicate with patients and/or families
- High level of interpersonal skills to handle sensitive and confidential situations
- Analytical ability and critical thinking to gather and interpret data
