* Mount Carmel Dublin Case Management facilitates communication and care coordination along the continuum through effective transitional care management utilizing a multidisciplinary team. ** Position
Purpose:
*
* The Emergency Department Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offer crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations.
This role will receive referrals for individuals from at-risk populations from Multi-disciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team). The Emergency Department Social Worker is responsible for lethality assessments and referrals to inpatient psychiatric facilities as indicated. In addition, the Emergency Department Social Worker will assist other Mount Carmel Health System Emergency Departments virtually through our Telehealth platform.
This position will primarily support the Emergency Department while still assisting on the inpatient unit as needed.
* This position is 12-hour shifts 8a-8p or 8p-8a. Available shifts may be any day of the week/weekend. Additional shifts can be picked up at Mount Carmel St. Ann's as well.

Education:
Master's degree in Social Work
· Licensure / Certification:
Current LSW license to practice as a social worker in the State of Ohio

Experience:
Minimum 1 year experience in a hospital setting with acute crisis intervention and behavioral health assessment experience.
· Effective Communication Skills
· Strong organization skills, self-directed, flexible, able to adapt to rapidly changing regulatory requirements.
· Possesses excellent clinical assessment and analytical skills.
· Computer competency and comfort with virtual telehealth platforms.
· Possesses effective interpersonal skills, can work across departmental boundaries, facilitates problem resolution, and maintain a professional demeanor in difficult situations.
· Knowledge of the functional operations of utilization management, discharge planning, care coordination and transitions of care
