Patient Navigator

Job Purpose:

- The Patient Navigator at Crescent Community Health Center will play a critical role in guiding patients through the healthcare system, ensuring they receive timely care and support. The primary purpose of this position is to assist patients in overcoming barriers to healthcare access, facilitate communication between patients and healthcare providers, and enhance the overall patient experience.

Key Responsibilities:

- Serve as a liaison between patients and healthcare providers, ensuring clear communication and understanding of medical procedures and treatment plans.

- Assist patients in scheduling appointments, coordinating referrals, and accessing community resources.

- Educate patients about their health conditions, treatment options, and preventive care measures.

- Identify and address barriers to healthcare access, including transportation, financial constraints, and language differences.

- Collaborate with healthcare teams to develop individualized care plans that meet the unique needs of each patient.

- Monitor patient progress and follow up on treatment adherence and outcomes.

- Advocate for patients' rights and needs within the healthcare system.

- Maintain accurate records and documentation of patient interactions and services provided.

- Participate in community outreach and education programs to raise awareness about available healthcare services.

- Continuously evaluate and improve patient navigation processes to enhance service delivery and patient satisfaction.

Required

Education:

- Bachelor's degree in healthcare, social work, public health, or a related field preferred.

Required Experience:

- Minimum of 2 years of experience in a healthcare setting, preferably in patient advocacy or case management.

- Experience working with diverse populations and understanding of community health resources.

- Proven track record of successfully navigating patients through healthcare systems and coordinating care.
