Locations- Hudson
- Shell Rock
- Washburn
- Winthrop
- Days, Nights, weekdays, weeknights, weekends
- Full‑time, Part‑time, PRN hours
- Flexible scheduling
- Up to 12‑hour shifts
Urgent / Immediate. May include:
- Medication administration
- G‑tube/enteral feedings
- Oxygen support
- Seizure management
- Trach and ventilator care
- Training provided
- Hourly wage: $2 per hour shift differential for 7:00 PM – 7:00 AM shifts
- Paid weekly benefits with full‑time hours
- Provide care based on licensure/certification within scope of practice
- Maintain an updated clinical record on each patient
- Evaluate effectiveness of nursing service to the patient and family
- Consult with physicians concerning changes of patient care plans
- Support FFHC’s mission by mentoring/coaching new employees during orientation
- Educate patients and caregivers on care needs
- Participate in QAPI program as requested
- Continuously improve nursing care skills
- Attend mandatory trainings/in‑services
- Submit required personnel file documentation timely
- Adhere to compliance and ethics expectations
- Maintain patient and family confidentiality
- Provide supportive, responsive interpersonal communication
- Perform other duties as assigned
- LPN/LVN Nursing Degree
- Unencumbered LPN/LVN License in state of employment or multi‑state license
- Minimum 1 year demonstrated experience
- Valid driver license with proof of insurance
- Clear background screening
- Working knowledge of Nurse Practice Acts
- Current CPR card/certificate with in‑person instructor‑led skills check
- Clear Hep B screening
- Clear TB screening
- Ability to read and communicate in English verbally and in writing
