• Administer prescribed medications and treatments per physician orders and delegation
• Monitor residents’ health status, document findings, and report changes to the supervising nurse
• Assist with activities of daily living and mobility to support comfort and safety
• Perform routine nursing procedures and basic wound care as directed
• Maintain accurate documentation in the electronic health record and ensure charting is up-to-date
• Collaborate with the care team to ensure quality, safety, and continuity of care

• Active LPN license in Iowa (or eligible to obtain licensure in Iowa)
• Current CPR/BLS certification
• Education:
  
  LPN diploma from an approved program
• Experience:
  
  Long-term care or geriatric nursing experience preferred

  Strong documentation and communication; ability to work in rotating shifts and adapt to a changing schedule
