SKIP partners with families and acts as a free GPS connecting children to the services, they need to grow up at home rather than in hospitals or institutions. A hospital is a great place to cure a child, but it is not a place for a child to live.

Job Description
The Community Habilitation Specialist provides instructions including adaptive skills development;
Hands‑on assistance with acts of daily living; community inclusions and relationship training and support for independence in travel and transportation;
Adult educational supports; development of social, leisure, self‑advocacy, and informed‑choice skills; and appropriate behavior development to help individuals access their communities.

- Work directly with an assigned participant(s) on the Habilitative actives specified in the individualized service plan.
- Assist the participant in skill development throughout implementation if residential habilitation plan.
- Assist in the planning and/or review the Community Habilitation outcomes along with the family, participant, family advisor and program coordinator.
- Submit progress notes and time sheets by entering information and hours worked on an agency provided tablet on the same day that services are provided.
- Follow all specified procedures, especially those regarding emergencies and incident reporting. Report incidents immediately.
- Must be 18 years or older.
- High School Diploma or GED.
- Prior experience working with people with developmental disabilities, mental health challenges, or in direct support roles (Preferred)
