Direction Home Akron Canton Area Agency on Aging & Disabilities is seeking a Care Management Intern to support care managers in LTSS. You will assist with care management for managed care enrollees, review assessments, and help determine service options.
You will also collaborate on Medicaid eligibility, enroll eligible consumers, and educate families through in-person visits or other channels. This role emphasizes advocacy, individualized care planning, and contributing to program goals.
