QUALIFICATIONS:
  • Education:

    Minimum Bachelor’s Degree in Social Work or related field required.
  • Licensure:
    Valid LSW or LPC preferred.

JOB SUMMARY:

The Independent Living Specialist, under the administrative/clinical supervision of the CIP Supervisor, provides case management and Community Support Program services to clients, their families, and/or significant others as needed. The Independent Living Specialist transitions clients in community settings and works to provide supervision, assistance to daily living, case coordination, guidance and transportation. The Independent Living Specialist is a member of a treatment team.

The Independent Living Specialist is expected to meet the program’s productivity target for billable service each week. Services take place in the home and community. Work hours are flexible, some evenings may apply.

ESSENTIAL DUTIES:
  • Accept assignment of cases from the supervisor to provide Community Independence Program Services for clients, their families, and significant others as needed, including: community organizations, schools, other health and social service professionals, the justice system, and other agencies involved with the client.
