Coordinator; Bilingual English​/Spanish

The START (Systemic, Therapeutic, Assessment, Resources, and Treatment) model serves people diagnosed with intellectual/developmental disabilities and co-occurring behavioral health conditions. This comprehensive model of service supports and optimizes independence, treatment, and community living for individuals with I/DD and behavioral health needs.

Under supervision of the Clinical Team Leader, START Coordinators support an assigned caseload of individuals, conducting evaluations and assessments both in-person and remotely, providing resources and coordinating with the interdisciplinary team to support individuals through crises and prepare for discharge. Regularly attends and coordinates meetings, ensuring fidelity to the START model and provides on-call support on a regular basis.

• Maintain an active caseload of up to 20–25 individuals, providing assessment, systemic consultation, outreach, and coordination of supports based on individual needs.
• Conduct regular in-person and/or telehealth outreach visits across home, day,
• Assist with referrals, enrollment/intake assessments, treatment and clinical referrals, and coordination with clinical and medical leadership as appropriate.
• Coordinate support meetings and develop crisis prevention/intervention plans using the START Action Plan, escalating concerns to the appropriate clinical team or leadership.
• Develop and maintain comprehensive service evaluations, intake and assessment documentation, intervention and outcome plans, crisis plans, and other required service records.
• Participate in regular consultation, staff, leadership, and clinical team meetings, as well as provide scheduled rotational on-call support.
• Maintain timely and accurate documentation, incident reporting, data entry, and required records in accordance with program, state, and regulatory requirements.
• Conduct outreach to community partners to establish and maintain collaborative relationships and service linkages.
• Develop and present materials for Clinical Education Team Meetings as assigned.
• Participate in the Center for START Services National Network, including virtual meetings, study groups, conferences, and other program events as appropriate.
• Complete all required trainings and maintain START Coordinator certification, along with compliance with YAI, NCSS, HIPAA, and applicable state/federal regulatory requirements.
• Perform other duties as assigned.

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field;
  and
• At least two (2) years of experience working with the I/DD population and/or behavioral health needs;
  or
• Satisfactory combination of education, experience and/or training as authorized by START/NCSS and/or appropriate State regulatory agency (e.g. OPWDD, etc.);
  however, all START Coordinators must have at least a bachelor’s degree in human services discipline.
• Successful completion of START Coordinator Certification training within 12 months of appointment, including demonstration of competencies in all associated areas.
• Ability to provide on-call coverage for crisis services on a regularly recurring basis, as designated by START leadership in assigned region beyond regular work hours including weekdays, weekends, evenings and/or overnight on a 24/7 basis, as required.
• Ability to travel regularly across assigned region(s) to conduct in-person assessments, evaluations and visits as required by caseload and to occasionally travel across other regions within state of operations and/or to other states as needed for NCSS meetings or events.
• Ability to work with complex systems and an interest in the population is essential.
• Exceptional interpersonal and problem-solving skills.
• Independent and organized with strong communication and writing skills.
• Highly flexible personality type and willingness to take initiative.
• Experience working with individuals with I/DD with co-occurring diagnoses and complex behavioral needs
• Experience working with people and their families and/or social service providers
• Verbal and/or written fluency in Spanish highly preferred; fluency in other language(s) may be preferred based on assigned region(s) and caseload.
