As a Home Health Aide, reporting to the Associate Director, Branch Administration at Center Well Home Health, you’ll help patients regain strength, mobility and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you’ll empower individuals to overcome physical limitations, perform everyday activities with confidence and enjoy a better quality of life.

Responsibilities
  • Provide direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
  • Assist the patient with self‑administered medications by opening bottle caps, reading medication labels, checking dosage against prescribed medication, and observing the patient taking medication.
  • Take accurate temperature, pulse, and blood pressure measurements and recognize and report abnormal results to supervisor.
  • Help patients maintain good personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care, nail/foot care, and other activities.
  • Assist in feeding patients, communicate basic principles of nutrition, observe and record food and fluid intake, and safely position patients for meals and support self‑feeding.
  • Assist with patient toileting including use of bedpan/urinal, change and position catheter bags, and perform bag change procedures on well‑regulated ostomies.
  • Provide necessary skills to safely assist the patient with mobility, exercises, positioning/turning, transfers, and ambulation per Plan of Care and Center Well Home Health policy.
  • Report changes and document pertinent information and care rendered to ensure continuity of care, documenting interactions with patients, caregivers, doctors, and other staff members appropriately, legibly, thoroughly, and within the allotted time.
  • Practice acceptable infection control principles and provide a clean, safe, and comfortable environment.
  • Help with other household tasks including light laundry, bed changing, bed making, light meal preparation, light housekeeping, and shopping if no other assistance is available and an MD order is present.
Qualifications
  • Complete Certified Nursing Assistant (CNA) or Certified Home Health Aide (HHA) program within the last 24 months.
  • Meet applicable state certification requirements.
  • Valid driver's license, auto insurance, and reliable transportation.
  • In good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare‑certified agency.
  • Minimum one year experience within the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
Additional Information
  • TB Statement:
    This role is considered patient‑facing and is part of Humana’s Tuberculosis (TB) screening program. You will be required to be screened for TB.
  • Driving Statement:
    This role is part of Humana’s driver safety program and requires a valid state driver's license with personal vehicle liability insurance meeting the minimum required limits or $25,000 bodily injury per person / $25,000 bodily injury per event / $10,000 property damage (whichever is higher).

    Hours:

    40 hours per week.
Pay Range

$37,440 - $43,800 per year
