The Medical Coordinator for La Jornada arranges and coordinates the proper delivery of medical, dental, mental health, and pharmacy services on behalf of children in care. This includes urgent, emergent and preventive health care. The Medical Coordinator accurately documents the services provided by maintaining complete and confidential records for children, and regularly communicates with children’s care teams and coordinates appointments and interpretation services.

- Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.
- Provide children with a positive and appropriate medical environment.
- Ensure confidentiality of health records and adhere to NYAP, State, and Funder guidelines for protection of PHI.
- Obtain Memoranda of Understanding (MOUs) with medical, dental, and mental health providers.
- Participate in intakes and discharges of children, including administering COVID‑19 testing, preparing medical documentation packages, and scanning/uploading documents.
- Continuously assess the children’s medical, dental, and mental health needs and coordinate care in accordance with NYAP and other policies.
- Collaborate with children’s assigned cross‑functional teams and providers to ensure appropriate care, including scheduling appointments, preparing TARs, transporting children, and obtaining medical clearances.
- Report medical emergencies via SIR/CLE process.
- Elevate issues of medical concern to the supervisor, team, providers, and DHUC as appropriate.
- Support foster parents by providing education on medical policies and assisting with required medical responsibilities.
- Collect, complete, and upload forms and health records into appropriate portals and systems.
- Maintain medicine cabinets and first aid kits with necessary supplies.
- Provide in‑service training and ongoing education to staff and foster parents on health‑related needs, medication rights, COVID‑19 testing, CPR, and First Aid.
- Maintain Red Cross CPR/First Aid Trainer’s certifications.
- Assist and support Program Leadership during auditing processes.
- Perform other duties as requested.
- Licensed Practical or Vocational Nurse (LPN/LVN) or higher health‑related qualification (e.g., RN with a bachelor’s degree) OR Bachelor’s degree in a relevant field.
- Bilingual fluency in English and Spanish.
- Minimum of 1 year of employment in public health or a healthcare setting demonstrating experience in care coordination, health administration, or direct clinical care.
- 1 year of experience working in the child welfare field with children and/or adolescents in a social service setting.
- Valid driver’s license, excellent driving record, safe and clean personal vehicle, and current automobile insurance.
- Ability to maintain compliance with agency‑required training programs.
- Proficient use of desktop and laptop computers, smartphones, tablets, printers, fax machines, and photocopiers, as well as software including word processing, spreadsheet, and database programs.
