• Provide direct care for all assigned patients, including initiating and maintaining communication with care team members.
• Evaluate and educate patients/care partners to safely administer home infusion therapy.
• Assess home environment to identify required interventions for safe care.
• Develop plan of care with team, evaluate response to therapy each encounter.
• Manage schedule via approved platform and promptly communicate any changes.
• Coordinate patient care via company platform.
• Participate in nurse on‑call rotation to ensure continuous support.
• Document all encounters, communications, and care activities at time of service per policy and regulatory requirements.
• Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
• Ensure timely submission of plans of treatment and required documents per policy and state guidelines.
• Collaborate with departments to ensure all required patient documents are completed and returned promptly.

• Full‑time, 40 hours per week, Monday to Friday, 8 am‑5 pm.
• Must participate in on‑call rotation: 1 weekday night per month; 1 weekend every 3 months on average.
• Must be knowledgeable of IV access devices (IV lines, ports, vial administration).
• Must routinely drive up to 1.5 hours, occasionally up to 2 hours (mileage reimbursement and hourly rate are offered).
• Must be licensed as an RN in Georgia.

• BSN or equivalent preferred; RN license in good standing.
• Advanced certification in specialty areas (CRNI, ONS, IgRN) preferred but not required.
• Minimum 3 years nursing experience with infusion therapy preferred; ICU or cardiac care experience preferred.
• Valid driver’s license with no significant restrictions.
• Proficiency with Microsoft Outlook and Office Suite.
• Must be available for on‑call rotation, including nights and weekends, and respond promptly to emergencies.
• Administrative oversight responsibilities: monitoring workflows, ensuring compliance with policies, managing resources efficiently.
