About the Role
As a Center Well Home Health nurse reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients return to the life they love.
Responsibilities- Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and their families on disease management, treatment options, and self‑care techniques.
- Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
- Maintain timely communication with patients, physicians, clinical manager(s), and other team members regarding patient or family needs and status changes. Participate in care coordination activities and discharge planning.
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
- Experience in a Home Health or Hospice setting.
Pay Per Visit/Unit Rate: $33.00 – $47.00.

Pay Range: $53,000 – $73,000 per year.
