Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years' experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
Roles & Responsibilities :
- Assess home health and hospice patients to identify and address their physical, psychosocial, and environmental needs.
- Develop, implement, document, and evaluate individualized Plans of Care to ensure quality and continuity of care.
- Educate patients and families on care practices, medications, diet, and home safety, ensuring methods are appropriate to the patient's needs.
- Supervise and provide clinical direction to Home Health Aides and other support staff to ensure a high level of service delivery.
- Monitor patient cases for compliance with third-party payer requirements and arrange referrals as needed.
- Maintain clear, concise, and current patient records, documenting all visits, contacts, and care plans accurately.
