Role Overview:
The Registered Nurse (Home Health) provides personalized, in-home nursing care to patients, promoting recovery, safety, and independence. Responsibilities include conducting assessments, delivering skilled interventions (wound care, infusions, medication management), educating patients and caregivers, monitoring progress, coordinating care with the healthcare team, and ensuring accurate documentation.
KeyQualifications & Skills:
Active RN license, at least one year of clinical experience, patient-focused, organized, with a valid driver’s license, reliable transportation, and CPR certification.
Key Duties- Serve as primary clinical partner for patients and families
- Perform comprehensive home assessments and develop care plans
- Administer skilled nursing interventions tailored to patient needs
- Educate and empower patients and caregivers
- Monitor patient progress and communicate with the care team
- Ensure care continuity and complete accurate documentation
