Home health travel assignment
- Administer skilled nursing care in clients' homes and coordinate care with the interdisciplinary team, patient/family, and referring agency; assess, implement, document, and evaluate the Plan of Care (POC) for quality and continuity
- Assess home health and/or hospice patients in stable and emergent situations; obtain data on physical, psychological, social, spiritual, and cultural factors and incorporate findings into the POC; maintain up-to-date patient records including visits, problems, plans, actions, and goals
- Provide education to patients and families regarding care, medications, home safety, and other relevant topics; use age- and culture-appropriate teaching methods and promote self-care practices
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure consistent, high-quality service and continuity of care
- Monitor and reassess pain; apply appropriate pain management techniques; coordinate referrals to appropriate agencies when needed; participate in interdisciplinary team meetings and maintain effective communication with patients, families, and health professionals
- Current state license as a Registered Nurse – Georgia, required
- 1-2 years RN clinical care experience; 2+ years home health experience required
- CHPN certification preferred; CPR certification required
- Education:
Associate Degree in Nursing or Nursing Diploma required;
Bachelor of Science in Nursing preferred - Proficiency with Homecare Homebase software and OASIS experience required
- Valid driver’s license with reliable transportation and insurance; ability to travel locally
- Knowledge of medications, IV care, wound management, catheter/drain care, and use of durable medical equipment; ability to work independently in home settings
- 40 hours per week guaranteed
- 13-week travel assignment with defined start date
