About the Role

As a Lavender Hour Aide (CNA) , you’ll provide compassionate, hands-on hospice care during the late afternoon and evening, typically 4:00 PM–9:00 PM , when patients and families may need an extra level of comfort, care, and reassurance.

The evening hours can be especially challenging as patients may become more tired, restless, or uncomfortable, while caregivers may need additional support. You’ll help patients with personal care, prepare them for the evening and bedtime, promote comfort and dignity, and provide a calm, reassuring presence for patients and their loved ones.

You’ll also observe and communicate changes in a patient’s condition to the hospice team, helping ensure concerns are addressed early and families feel supported.

What You'll Do

Responsibilities may include, but are not limited to:

  • Provide personal care in accordance with the patient's individualized plan of care and Hospice Aide Care Plan.
  • Assist with bathing, grooming, oral care, toileting, incontinence care, repositioning, dressing, and other activities of daily living.
  • Assist patients with preparing for bed and settling comfortably into the evening.
  • Provide skin care and other comfort-focused interventions within the scope of the role.
  • Respect each patient's dignity, privacy, preferences, and individual routines.
Promote Comfort & Quality of Life
  • Provide comfort measures that support the patient's individualized plan of care.
  • Maintain a clean, safe, calm, and comfortable patient environment.
  • Recognize nonverbal signs of discomfort, distress, or changes in condition.
  • Offer a calm and reassuring presence to patients and families during the evening hours.
  • Provide additional support when patient needs or caregiver fatigue may increase.
  • Observe and promptly report changes in physical condition, behavior, comfort, skin integrity, mobility, intake, elimination, or overall status.
  • Immediately communicate concerns related to pain, breathing difficulty, agitation, falls, safety, or other significant changes to the appropriate nurse.
  • Reinforce education previously provided by the clinical team within the scope of the role.
  • Work closely with the RN case manager, on-call nurse, and interdisciplinary hospice team.
  • Communicate important observations and changes to support coordinated patient care.
Support Patients & Caregivers
  • Help patients transition comfortably into their evening routine.
  • Provide reassurance and support to caregivers as they prepare for the overnight hours.
  • Assist with basic household tasks related to patient care, as appropriate.
  • Help reduce caregiver stress by addressing appropriate patient needs proactively.

    Follow infection prevention, safety, patient privacy, and regulatory standards.
  • Complete accurate and timely documentation of all care provided.
  • Communicate missed, delayed, or changed visits promptly.
