Position: Hospice Social Worker - Full Time

Targeted pay range: $33.00 - $38.00 per hour and an exempt/salary position.
Supports our Hospice Duluth and North Branch teams.
Full‑time .8 FTE (64 hrs bi‑weekly). Weekdays 8:00‑4:30 pm – no weekends, no holidays, and no on‑call requirements.
Pay determined by experience, qualifications, certifications, licenses, and education.

Essential

Job Responsibilities

Perform initial and ongoing assessment of the psychosocial status of terminally ill patients and families/caregivers related to the patient’s condition and environment. Provide community resources, short‑term anticipatory grief counseling, emotional support and end‑of‑life education.

Assess the family/caregiver’s bereavement risk and provide appropriate resources/interventions.

Monitor and assess family dynamics, caregiver ability to provide safe and effective care, communication patterns, backup planning for alternative living situations, and safety issues such as neglect or abuse. Develop interventions as warranted.

Participate in developing individualized care plans, involving the patient and family. Attend interdisciplinary group meetings and support the team in recognizing psychosocial stressors. Maintain clinical records for all patients referred to social work.

Assist patients and families/caregivers in planning funeral arrangements; provide resources for financial, legal, and healthcare decision responsibilities.

Facilitate care conferences with the hospice team and family to ensure open communication and understanding of hospice philosophy, services, and the dying process. Provide bereavement follow‑up per agency protocol.

Coordinate Mosaic Moments, nursing home/residential hospice placement, respite stays, and travel contracts as needed.

Driving Requirements

Local travel and driver’s license required.

Minimum

Required Qualifications

Master’s Degree in social work.
Licensed Graduate Social Worker in good standing in Minnesota.
Two or more years of social work experience in a health care setting.
Valid driver’s license, proof of insurance, and satisfactory motor vehicle record.
Ability to communicate effectively in both written and verbal formats.
Ability to follow all safety rules, regulations, policies, and procedures of the facility, including but not limited to Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to Know, SDS, ADA, HUD Policy and Procedures, and Fair Housing laws and regulations.

Preferred Qualifications

LISW or LICSW license.
Previous experience working with seniors.

Licenses & Certifications

Licensed Social Worker (required).
