  36 hours per week, with a flexible Monday through Friday schedule.
• Settings: Outpatient, Skilled Nursing Facilities (SNF), Long-Term Care (LTC), and Home Health care services.

Our skilled clinicians work in collaborative care teams with energy and passion for providing the highest level of excellence in patient care. Offering PT, OT, and SLP services, we incorporate a multidisciplinary approach with great teamwork all around.

Mahnomen is situated in a scenic part of Minnesota, surrounded by forests, lakes, and wetlands. The area is rich in natural beauty, with opportunities for outdoor activities like fishing, hiking, and birdwatching. Mahnomen itself lies on the shores of Lake Maud, adding to its appeal for those who enjoy nature and water-based activities.
