Certified Nurse Assistant; CNA Per Diem

Job Summary

Become a part of our caring community as a Home Health Aide (CNA). Report to the Associate Director, Branch Administration at Center Well Home Health and help patients regain strength, mobility and independence from the comfort of their homes.

• Provide direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist patients with self‑administered medications by opening bottle caps, reading medication labels, verifying doses and observing ingestion.
• Take accurate temperature, pulse and blood pressure measurements; recognize and report abnormal results.
• Help patients maintain personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care and nail/foot care.
• Assist with feeding, communicate basic nutrition principles, observe and record food and fluid intake, and safely position patients for meals.
• Support toileting, including use of bedpan/urinal, catheter bags and ostomy care procedures.
• Assist with patient mobility, exercises, positioning/turning, transfers and ambulation as per Plan of Care and Center Well Home Health policy.
• Document all interactions, care rendered, and pertinent information to ensure continuity of care.
• Practice infection control principles and maintain a clean, safe, and comfortable environment.
• Perform light household tasks such as laundry, bed changing, light meal preparation, light housekeeping, and shopping when no other assistance is available and an MD order is present.

• Completion of a Certified Nursing Assistant (CNA) or Certified Home Health Aide (HHA) Program within the last 24 months.
• Meet all applicable state certification requirements.
• Valid driver’s license, auto insurance, and reliable transportation.
• Good standing on the HHA Registry (if applicable) and completion of the HHA/CNA course for Medicare‑certified agencies.
• Minimum one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health, or hospice agency.

TB Statement
:
This role is patient‑facing and is part of Humana’s Tuberculosis screening program. Screening for TB is required.

Driving Statement
:
The role is part of Humana’s driver safety program; a valid state driver’s license, personal vehicle liability insurance, and compliance with state minimum limits or specified dollar amounts are required.

Pay Range
: $37,440 – $43,800 per year (for full‑time 40‑hour weeks; may vary by location and qualifications).
