Moments Hospice is seeking a Hospice Social Worker to travel to new locations, helping each site pass survey readiness and get up and running. The temporary assignment typically lasts 13–26 weeks, offering autonomy and variety in hospice care.
As part of a dedicated team, you will provide psychosocial support to terminally ill patients and families, manage case loads, and coordinate care with the interdisciplinary team to ensure compassionate end-of-life services.
