Residential Transitions, Inc. (RTI) is seeking a Targeted Case Manager to assess needs, develop service plans, and connect clients with community resources for individuals with serious mental illness and co-occurring conditions in Minnesota.
You will document services, coordinate with the team, and support clients to live independently at home and in RTI's residential settings. A valid driver’s license and auto insurance are required.
