Summary:

Care Coordinators maintain responsibility coordinating and providing services for adults/children who have been diagnosed with a mental illness. This position works with a multi-disciplinary team of internal and external providers.

• Accept assigned referrals and engage clients to initiate services.
• Help navigate internal and external health services.
• Assist in the client's integrated treatment coordination, which may include treatment planning, completing release of information(s), progress notes, and referrals to internal and external providers.
• Coordinate client services in areas of mental health, chemical dependency, housing, financial benefits, insurance benefits, employment, transportation, education, and other basic needs.

• Two or more years of experience providing mental health services to clients.
• Experience navigating health, employment, housing, and/or insurance systems.

• Supervisory experience.

• Flexible Scheduling
• Paid Supervision during licensure.
• Extensive training and certifications, including EMDR, TF-CBT, DBT, ECMH, and many more.
