Cook Hospital & Care Center is seeking an experienced healthcare professional to serve as our Outreach Manager
. This position is responsible for the coordination, development, and day-to-day oversight of the hospital’s outpatient and outreach services, helping ensure accessible, high-quality services for patients throughout our community and surrounding region.

The Outreach Manager oversees a diverse portfolio of outpatient services, including Telemedicine, Specialty Clinics, Outpatient IV Infusions, Outpatient Procedures, Cardiac Rehabilitation, Pulmonary Rehabilitation, and other outreach services
. This position works closely with physicians, providers, clinical departments, staff, and external healthcare partners to coordinate services, develop programs, improve patient access, and support continued growth of outpatient care.

The Outreach Manager also serves as the hospital’s Trauma/Stroke Coordinator
, supporting program coordination, quality improvement, regulatory requirements, education, reporting, and collaboration with regional partners.

Key Responsibilities

Outpatient & Outreach Services

  • Oversee and coordinate assigned outpatient and outreach services, ensuring effective day-to-day operations and a positive patient experience.
  • Coordinate Telemedicine services, including scheduling, equipment, provider coordination, staff training, and related operational needs.
  • Coordinate and support Specialty Clinics, working with visiting specialists, providers, staff, and patients to ensure efficient clinic operations.
  • Oversee Outpatient IV Infusion and Outpatient Procedure services, including coordination of scheduling, staffing, supplies, equipment, and patient care processes.
  • Oversee and support the development and operations of Cardiac and Pulmonary Rehabilitation services.
  • Identify opportunities to expand outpatient services and improve access to specialty and regional healthcare services.
  • Collaborate with physicians, providers, clinical departments, and external healthcare organizations to establish and maintain effective service relationships.
  • Monitor program performance, patient volumes, utilization, and outcomes and identify opportunities for improvement.
  • Participate in budgeting, purchasing, equipment planning, and resource management for assigned services.
  • Develop and maintain policies, procedures, workflows, and processes necessary for effective program operations.
  • Ensure services are delivered in accordance with applicable regulatory, accreditation, and organizational requirements.
Cardiac & Pulmonary Rehabilitation
  • Coordinate the overall operation and development of Cardiac and Pulmonary Rehabilitation services.
  • Coordinate patient referrals, evaluations, treatment plans, exercise programs, monitoring, education, and follow-up.
  • Monitor patients during rehabilitation activities, including appropriate assessment of vital signs, heart rate, blood pressure, respiratory status, and EKG monitoring as applicable.
